Clinical Nurse Navigator - Complex Management 1.0 FTE Fixed Term 2 years

Are you a Nurse who is looking for change but still be able to use your clinical knowledge without the hands on clinical aspect.  This role would suit someone with an interest in primary care who is able to use their Nursing experience and knowledge to influence patient outcomes through a collaborative approach in developing MDT processes across complex management and care coordination.

The Position:

This role will see you working across Northland within localities and with the wider LTC team, to assist the implementation of changes and building on already established effective models to improve health outcomes for our Northland population.  The focus of the role will be on the development of MDT processes and embedding MDT’s into everyday practice with ongoing refinement of pathways that support engagement and improved patient outcomes.  This role will work to provide clinical advice and support to community and primary care nurses and other providers to build the workforce capability and raise the level of expertise for MDT complex management.

To be considered for this position you ideally will have:

2-3 years of Nursing experience in a NZ Healthcare environment
A current Registered Nurse practicing certificate
Evidence of advanced learning/qualifications applicable to the position
Proficient Computer literacy skills, e.g. Microsoft Office programmes.
Current and Un-encumbered Drivers Licence
Proven interpersonal skills in developing relationships

 

You must have a current NZ work visa and the legal right to work in NZ.
