Supporting change and improving Primary Health service delivery

Mahitahi Hauora supports Primary Healthcare providers including General Practices and Māori Health Providers to deliver care and to look after the wellbeing of their communities and whānau. We also deliver care and wellbeing support directly to the community and whānau.  As a Mahitahi employee you will be passionate about supporting transformational change and improve service delivery in primary health.

Mahitahi Hauora follow the kaupapa of “What Matters to Whānau”, working to the framework of Papa Tikanga.  Together, achieving What Matters to Whānau means engaging and listening to whānau voice to inform service design and delivery while also supporting community and whānau-driven solutions that promote wellness.

Neighbourhood Healthcare Home Background
NHH is the Northland version of the national Healthcare Home (HCH) model of care which incorporates LEAN methodologies to improve access to primary care services in order to reduce use of hospital services.  The model of care is grouped into four core domains:

1.      Improved sustainability (When I visit the practice)
2.      Ready access to urgent and unplanned care (When I’m unwell)
3.      Proactive care for those with more complex needs (To help me stay well)
4.      Better routine and preventative care (To keep me healthy)

The enhanced HCH model of care, published in October 2020, aligns with Pae Ora (Healthy Futures) as a vision and a new set of values with an explicit focus on improving equity and consumer involvement.

About our Role

We are looking for an Equity Improvement Partner to join our team to champion the improvement of equity and consumer involvement within the enhanced Pae ora (Healthy Futures) model of care through working with our primary healthcare providers in implementing our Neighbourhood Healthcare Homes (NHH) programme.

As an integral member of our improvement team, you will be responsible for bringing a strong equity lens in applying competent skills, knowledge and experience of improvement and change methodologies in facilitating solutions to system problems that improve equity for Māori, quality, safety, productivity and efficiency utilising the NHH Change Management Framework. This position is a specialist NHH position and will support NHH primary care service providers to deliver through the NHH model of care enabling a quality and improvement led focus in delivery of primary care to our enrolled population.

Ideally you will have worked in or have an understanding of primary health, however if you are passionate about improving Māori health outcomes in primary healthcare service delivery, supporting practices and providers that are delivering these services to whānau, then this opportunity could be for you. 

This role is a relationship driven role, and would suit someone who is passionate about Māori health and wellbeing of whānau and enjoys an autonomous role while also being an integral part of a team.

The role requires someone with:

  • The ability to develop strong relationships with health professionals, clinicians and partners in primary care
  • Experience in facilitation of organisational change, and implementation of new strategies and initiatives
  • Education or qualifications related to quality, change management strategies, improvement methodologies or frameworks.
  • Proven ability to engage with and influence people to adopt new models or systems
  • High level of IT competency including MS suite
  • Excellence in verbal and written communications
  • A commitment to quality improvement in general practice and primary healthcare 

You must have an unencumbered NZ Driver Licence and the legal right to work in NZ.  Health professionals will be supported to maintain their Annual Practising Certificate.
