Mahitahi Hauora PHE - Kaiarahi Manawaroa - Long Term Conditions

We are looking for someone who is whanau and community focussed, has a desire to work in and support the health and wellbeing of whanau.  This person will assist with direct whanau support, linking whanau and providers, identifying service gaps and building on already established effective models to improve health outcomes for our Tai Tokerau population.  This role has a particular focus in supporting those with diagnosed or at risk of developing Heart Disease, Gout and Diabetes to support self-determined wellbeing and access to appropriate resources and services.  The Kaiarahi role will provide support in a holistic way to whanau, community and providers to build and strengthen community relationships and assist in improving equity of outcomes for Maori.

Mahitahi Hauora is a primary health entity established to transform how primary healthcare is delivered in Northland.  Our organisation operates as a Trust, is not for profit and works to support a primary health care system that sustains equitable self-determined wellbeing and ensures every person in Northland can live a long and healthy life. 

The Position

We are looking for someone who has strengths in the following:

  • Ability to advocate and navigate the primary and community healthcare system supporting whanau living with diabetes, gout and or heart conditions
  • Ability to work autonomously and as part of a team
  • Knowledge of Northland communities and services
  • A confident communicator
  • Understanding of Maori tikanga and models of care eg Te Ware Tapa Wha

To be considered for this position you ideally will have:

  • A good level and understanding of English
  • Demonstrable experience in engaging with community and growing collaborative support
  • Proven interpersonal skills in developing relationships
  • Some ability to communicate and understand Te Reo
  • Current and Un-encumbered Drivers Licence
  • Proficient Computer literacy skills, e.g. Microsoft Office programmes.

You must have a current NZ work visa and the legal right to work in NZ.
