Mahitahi Hauora PHE - Primary Mental Health & Addictions Clinical Coordinator

Mahitahi Hauora is a primary health entity established to transform how primary healthcare is delivered in Northland.  Our vision is to support a primary health care system that sustains equitable self-determined wellbeing and ensures every person in Northland can live a long and healthy life.

We are seeking an enthusiastic and motivated person to join our Primary Mental Health Clinical Services Team to co-ordinate and deliver a Primary Health Care service for clients/whanau with mild to moderate mental health issues.

This primary health care service will involve case management, assessment, brief intervention and referral and interaction with other community services, including support to general practice and data collection and analysis.

 For this role we are seeking someone who has:

  • A current New Zealand registration as a health professional in Nursing or Allied Health
  • A current practicing certificate.
  • Has or is working towards relevant post graduate qualifications.
  • At least 5 years recent experience as a mental health and/or addictions practitioner.
  • Proven experience working with both adult and youth, and passionate in improving service accessibility and supporting those with mental health challenges.
  • The ability to proactively develop and maintain positive relationships across all primary and secondary health sectors, including community providers.
  • Demonstrable ability to oversee the implementation and delivery of an innovative model of care
  • Experience in brief opportunistic interventions and brief therapy
  • A current unencumbered full NZ driver’s licence
  • Excellent computer and literacy skills including MS Office Suite
  • Excellent verbal and written communication skills        

You must have the right to work in NZ and/or have a current NZ work visa.
